Quantification of cerebral perfusion SPECT images based on MRI co-registration and anatomic brain atlas

Abstract

The quantification of the regional cerebral blood flow (rCBF) by SPECT presents different limitations related to the poor spatial resolution, the selection of the regions of interest (ROIs) and the selection of the reference region used for the relative quantification. The objective of the present work is to propose a methodology for the relative quantification of the rCBF from the co-registration of the SPECT with the anatomical magnetic resonance image (MRI) and using anatomical atlases for the definition of the ROIs. The methodology includes three main steps: spatial pre-processing, partial volume correction, and calculation of the perfusion index (PI) and asymmetry index (IA). The methodology was implemented in a total of 27 patients with different disorders: 17 patients with epilepsy and 10 patients with Parkinson's disease (PD). Different anatomical atlases were used for each of these disorders. In patients with epilepsy, the results showed an increase in PI and AI in the ictal study compared to the interictal study in the regions corresponding to the epileptogenic zone (EZ). In PD patients, regions with significant differences in both PI and IA were identified between patients at different stages. The quantification of the rCBF using atlas allowed better anatomical definition of the regions corresponding to the EZ. Quantification of rCBF in patients with PD made it possible to correlate functional changes with disease progression. The methodology for the quantification of rCBF from SPECT, MRI and anatomical atlases is offered as a tool for the identification of EZ in the pre-surgical evaluation of patients with epilepsy and for the evaluation of the severity of the neurodegenerative framework in PD.

Resumen

La cuantificación del flujo sanquíneo cerebral regional (FSCr) por SPECT presenta distintas limitaciones relacionadas con la pobre resolución espacial, la selección de las regiones de interés (ROI) y la selección de la región de referencia utilizada para la cuantificación relativa. El objetivo del presente trabajo es proponer una metodología para la cuantificación relativa del FSCr a partir del co-registro de la SPECT con la imagen anatómica de resonancia magnética (IRM) y utilizando atlas anatómicos para la definición de las ROIs. La metodología incluye tres pasos principales: pre-procesamiento espacial, corrección del volumen parcial, y cálculo del índice de perfusión (IP) e índice de asimetría (IA). La metodología se implementó en un total de 27 pacientes con distintos trastornos: 17 pacientes con epilepsia, y 10 pacientes con enfermedad de Parkinson (EP). Para cada uno de estos trastornos se utilizaron distintos atlas anatómicos. En los pacientes con epilepsia, los resultados mostraron un incremento en IP e IA en el estudio ictal en comparación con el interictal en las regiones correspondientes a la zona epileptógena (ZE). En los pacientes con EP se identificaron regiones con diferencias significativas tanto en IP como IA entre pacientes en distintos estadios. La cuantificación del FSCr utilizando atlas permitió mejor definición anatómica de las regiones correspondientes a la ZE. La cuantificación del FSCr en pacientes con EP permitió correlacionar  cambios funcionales con la progresión de la enfermedad. La metodología para la cuantificación del FSCr a partir del SPECT, IRM y atlas anatómicos se ofrece como herramienta para la identificación de la ZE en la evaluación pre-quirúrgica de pacientes con epilepsia y para la evaluación de la gravedad del cuadro neurodegenerativo en la EP.
